How do you get the penny in after you have printed it? I've tried and tried...

I used a soldering iron (preferably with a thicker chisel-like tip) to heat up the coin and melt it into place.

However, I had the penny in my bottle opener snap so you might want to consider using another coin. Possibly a pre-zinc core penny (pre-1982) might hold up better.

do you have a design file you used to make this? It would be helpful to have the shape of the negative space to be able to make anything into a bottle opened?

We made this for UK monies. A five pence piece to be exact. We paused at the first layer that covered the coin and had to remove this layer (above the coin only) for the coin to fit. We then put a layer of acetone on top of the coin to aid with the next pass sticking properly. Worked really well and have already had many beers aided by it.

What's the diameter of a penny? So us ungodly foreigners can find an appropriately sized coin/washer. ;)
