This is a case for the RAMPS LCD Controller from RepRap Discount myworld.ebay.de/reprapdiscount/
Instructions
The two parts are assembled using 4 small screws that go through the PCB into the top part and fix the PCB and display in place.
To mount the case to your machine either simply put screws through the walls (they are a bit thicker than would be neccesary otherwise) or modify the case to your liking.
