I didn't write this script-- it comes from Swineworld.org, a Blender graphics site. A full description of the functionality of this extremely useful script can be found here:

If I get a chance I'll post a tutorial on compound gears like the 4:1 Rome Gear, but the tutorial on Swineworld is quite excellent for the basics of building gears.
Instructions
Put the script in your Blender scripts directory (.blender/scripts) and start Blender. Under the add: mesh menu you'll now have an option to "add gear" which does exactly what you'd think!
EDIT: The script doesn't fill in all the geometry of the gears-- you'll need to do some extra work to turn them into solid geometry. This tutorial should tell you what you need to know:
