Printed and tested several LM8UU holders, this one does not break _and_ is unobtrusive. Used it in an X-carriage mod. 8-)

Instructions

Use a block of insulating wood between the bed and the holder to prevent the holder from softening.

Again, use a cable tie to make sure the bearing is held firmly.

I use 3 of these for to support the bed.

The holder is attached to the block with 2 countersunk wood screws.

The block is attached to the bed with a single countersunk wood screw.

This means that it is self aligning and there is no need for precision drilling.

I believe that you are missing a SCAD file here; the file gregs-y-axis-holder.scad refers to gregs-lm8uu-holder.scad, but I cannot find it.
