What size nozzle are you using? My slicer wants to skip most of the raised text because it's too thin for my .4 nozzle.

What are the two diameters (small and large) for? Do I need to design my holes with one of those depending on the material or top of the hole with large, bottom with small? Would be great if you could explain that a little more. Also the metric file seems to need to be scaled by factor 25.4 if the slicer works in Millimeter.

Correct, you need to design your holes with a taper to accommodate the required taper and depth of the desired heat insert. The smaller diameter is the root of the taper, the larger diameter is the opening of the taper, the thickness is the minimum thickness or the depth of the tapered hole. Search McMaster Carr website for heat insert and you will see that the numbers on the 3d model match up to their design table of their available inserts. Sorry about the metric scaling. I forgot to set the Inventor stl options to metric. I have added in a metric Rev1 file with proper scaling. Good questions; thanks!
