Does the knife itself is pushed down by a spring?
I ask as even with a flat surface like a mirror it would require to be perfectly leveled so that the cut would be uniform.

In a 3D printer for example you can adjust the bed level, but I'd like to use this on a mini CNC and I have no way to adjust the bed.

The mount is the spring, if you have a close look at the vid you will see it move.

Does the holder have bearings for swiveling?

I'm assuming this is printed in PLA...but how many perims and infill? I assume it's designed to have some flex to it so the perims and infill could make a big difference in how much flex and I'm guessing how well it works. I don't have a drag knife yet or any experience with them so I'm just making a bunch of assumptions here and would like to minimize the trial and error ;)

It shouldn't matter much as long as you get some sort of flex to minimize surface imperfections.

The only part that will move is the thin sections and that is all going to be determined by your nozzle size. You can slice it and change settings and it should still give you the same thin sections.
