Two questions:

What’s the hole at the bottom of the larger gear for?

The big gear ring on my print seems to be larger than the area taken up by the three smaller gears. Does that ring need to be printed at 100% or slightly smaller?

Your first question likely refers to the sideways hole in the tallest of the small gears. This hole is for an M3 screw to tighten the gear against the rotating metal rod coming out of the music box. Without this, the gear would rotate freely on the rod.

The big gear ring may be too large if your printer is finely tuned. At the time I was having a hard time with "elephant foot"-- the first print layer squishing out slightly and impeding gear motion. I may have made it slightly larger than ideal to compensate, but I can't remember. Try printing it 99% and see what happens.
