Summary
Thin profile whistle, originally a pendant design but adapted to be printed using FDM printers like the Up! or the Makerbot.
... Sorry about the logo/URL on the back, feel free to remove it. I'll upload a clean version soon [once I boot back into windows].
Edit : Uploaded a 'WTF' copy instead :P Please print one, colour it in and post a pic? :)
Can someone with a MakerBot please try print this and tell me if it is OK? I've tested on the Up! and its very loud :)

Instructions
For most FDM printers, to minimize the need for support structures, prient the model onto its 'side' [thin long side] before printing. I uploaded a photo of this orientation, the 'support raft' is the bottom.
For the Up! (PP3DP) printer, set your support raft options to (less than) 40deg and (less than)10mm.
Then print and blow!
//Finally figured out why my instructions were sometimes truncating...
